| Complete and return the THS ETHOS, a statement of compliance to the
principles and standards upon which The Health Store operates. Self
certification is required on an annual basis. |
| Do not forget to include your shareholding cheque. |
| Your store(s) will be inspected to ensure it meets the standards of the
THS Ethos. |
| Your application and references will be passed to the Committee of
Management for approval. |
| New members can be trading within seven days. |
| Should you resign for any reason, such as retirement or store closure your
shareholding will be fully redeemed (assuming there are no outstanding debts
to the group). |
